I have been having the same problems for a long time. First try these tips from riot. Second, play around with your resolution in game to see if that helps. These first two things seemed to correct for 90% of the problem for me. But I would still occasionally have problems.

LoL says in the Summoners Rift video "We want you to fight the enemy team, not the game". With the current in game mouse cursor, it feels as though I am fighting the game and I have a difficult time trying to play. The mouse model could be important, if it has a button on it that changes the DPI and you're accidently pressing it. Like most modern "gaming" mice. I have a "sniper" button right where my thumb is resting on my mouse, which cuts my mouse's speed to 1/10 its normal speed. Could easily be pressed in the heat of gaming.

Seems to be a longtime bug in the game where having two monitors will cause your mouse to leave the league of legends window. Pressing F9 twice fixes it. One press will give league a red border and forces it into a mode where your mouse is supposed to be able to leave the window, pressing it a 2nd time disables this mode and locks your mouse
